The Registrar will Require the following Documents

  • The Medical Certificate of Cause of Death (if the death has not been referred to the coroner)
  • If easily available
  • The deceased’s Medical Card, Birth Certificate and, if applicable, Marriage Certificate
  • Any book or document relating to any pension or allowance the deceased received from public funds
  • You may not need to produce all of these documents if you can provide the necessary information. Please check with the Registrar before obtaining documents or copies if you do not have them to hand.

You will most likely need to make an appointment with the Registrar. We can advise you on this if you have any concerns.

Documents issued by the Registrar

The Registrar issues the Certificate of Burial or Cremation (Green Form). You should give this to the Funeral Director, who will forward it to the crematorium or cemetery. A White Certificate, form 344, is also issued for the DWP. The Registrar does not charge for the above certificates.
For a small charge, you may obtain certified copies of the Death Certificate for insurance companies, banks, building societies, employers etc. We recommend that you obtain copies of the death certificate at the time of registering, as some registrars charge extra to make duplicates at a later date.
